Consent & Agreement
1. Purpose & Nature of Services
I understand that I am participating in wellness-based services provided by a licensed physical therapist through Frontline Wellness and Recovery. These services are intended to promote general health, strength, mobility, and recovery. They are not designed to diagnose, treat, or manage medical conditions.
Services may include:
- Resistance Training
- Dry Needling (wellness-focused, non-clinical use)
- Soft Tissue Mobilization
- Instrument-Assisted Soft Tissue Mobilization (IASTM)
- Cupping Therapy
All services are voluntary and tailored to personal wellness goals and tolerance.
2. Acknowledgment of Risks
I understand that wellness services and physical activity carry inherent risks, which may include:
- Muscle soreness or fatigue
- Temporary pain or discomfort
- Skin irritation or bruising (especially from dry needling and cupping)
- Aggravation of prior conditions
I agree to communicate any medical concerns, medications, or changes in health to the provider. I accept responsibility for any adverse effects that may occur, and I release Frontline Wellness and Recovery and Francis Lacap, DPT, from liability, except in cases of gross negligence.

3. Consent to Participate
I voluntarily consent to receive wellness services and agree to participate in the program offered by Frontline Wellness and Recovery. I understand that I may decline or withdraw from any service at any time. I acknowledge these sessions are not clinical physical therapy or medical treatment and are not billable to
insurance. If any medical need arises, I will be referred to a licensed healthcare
provider.
4. Payment Policy
Packages and sessions are non-refundable and must be used within timeframe listed on package.
Payment is required to book and secure appointments.
5. Referral Program
Frontline Wellness and Recovery offers a referral program:
Refer a friend, and if they purchase a package, you will receive one free session.
Limit one free session per referred client. Referral must be mentioned at the time of booking.
6. Cancellation Policy
Cancellations or reschedules must be made with at least 24 hours' notice. Late cancellations may be charged the full session fee.
If you arrive more than 15 minutes late, the session will be canceled and marked as a no-show. No refunds will be issued.

7. Text Message Consent:
By booking with us, you agree to receive text messages regarding appointment updates and reminders. Message and data rates may apply based on your carrier.
You may opt out at any time by replying "STOP". To resume messages, reply "UNSTOP".

8. Confidentiality
All information shared is confidential and will be handled in compliance with HIPAA and applicable privacy laws. No personal or medical information will be disclosed without written consent.

9. Management Rights Consent:
By using our services, you acknowledge and agree that management reserves all rights to modify policies, services, pricing, communication methods, scheduling, and availability at any time without prior notice.

I have read and understood the information above. I consent to participate in wellness services with Frontline Wellness and Recovery. I understand the nature, purpose, risks, and payment terms of these services. By signing and booking, I agree to all above terms and conditions.
